Banks are a dime a dozen. The US is littered with banks. If you are concerned, change banks.
Yes, changing banks is a possibility.
Why are you worried about someone learning your bank account balances?
Maybe this is a good reason to NOT invest with a bank?
Is this going to be different for any bank, or even at Vanguard?

If you're worried, when you login to USBank, click "profile & setting", then open the security center and activate everything.
As stated in the OP, most US Bank customers won't care. I'm sharing this so affected people can be aware. Such as: battered victims hiding from abusive partners. They can be aware that phone CSR security is not strong/unique; their abusers could take advantage of that to find out where charges are routinely made and locate them.

The online "profile & setting" does not limit phone CSR. Phone CSR said the basic information listed in the OP is all that is needed to be verified on a call.

I went into a US Bank branch to talk to the New Accounts Rep to discuss this. He called the security department to ask if text codes could be activated, just like online banking, but was told this is not a feature for phone contact. So, I followed the actionable steps in the OP: I opened a new account and changed mother's maiden name.

Changing mother's maiden name to one that's unknown to others protects from unauthorized phone inquiries. The phone CSR directs any caller who fails the mother's maiden name question to go into a branch.
